Like many I didn't recieve the code for my radio when I bought the vehicle. I thought I would pass on some stuff I learned:
Call 888-Mitsu2005. Press 1, then 4. Hours are M-F 6am-5pm PST. They asked for my VIN, Radio Model Number and radio serial number. Asked for my name and address. If you are legit, this is the fastest way to get your code. This service is free!

. You don't need to provide your VIN, just radio model number and radio serial number. He seems to answer your request within 24 hours (usually less). He does have an email address which you can find searching the thread (out of respect for his time and inbox, I'm not going to post it here). To avoid wearing out our welcome, I would urge you to use option 1 first.

If you don't like these options, try one of the other methods on this site.
If you don't know how to find your radio model number and serial number, search this site as well. The short version is remove the bezel around the radio by carefully prying it out. I used a small putty knife with electrical tape covering the blade (to prevent scratches) to get mine off. Then remove the four screws and the radio should come right out. Model and serial number are on the radio. Some say you can see the numbers without removing the radio.
