Default Question about trans noise??

I’m helping a guy down the road from me fix up a VR4 he bought last week. Well when he first started it after bringing it home it made a knock so we shut it down thinking it was the top end. The next day we started it again and found out the noise went away when the clutch was pressed in. so we pulled the trans tonight thinking it was a backed out bolt, bad TOB, or a broke gear in the trans. Well all the bolts were tight and I spun the trans through all the gears and it didn’t feel like there were any bad gears.

After a while of looking around thinking we wasted time pulling the trans I noticed the clutch, and this is what I saw.(picture below) I don’t really understand how something could be hitting the springs and rivets while the clutch is engaged, if anyone has any input of how this happened and what we should do to prevent this from happening again the would be great. I know he ordered a new clutch and he is getting the flywheel resurfaced.

I'm heading over to the trans shop in about 30 min to have them look at it.
Edit he said it's probably a bearing inside the transmission....
