It looks like the gears are a little worn. I'll bet your drain plug was just full of metal bits and there may have been silver fluid in your oil when you drained it.
This is just a guess but I would bet your bearing preload is too loose on the intermediate shaft. An easy way to check it is to remove everything out of the transmission, put just the intermediate shaft in the case and tighten down all of the bolts. Then go in through the hole where the gear select lever goes in and spin the shaft with a screw driver. It should have some resistance and should stop quickly after you stop pushing it. If it spins freely, there is not enough preload on the bearings. What happens in that case is that when the transmission heats up, there will be even more play in shaft. It will move slightly sideways away from the input shaft so the gears will not make a good contact and it will eat up the gears.

The gear on my input shaft looks like this too. very worn and "scalloped" I would like to know if that means I should replace the shaft as well. Doesn't look like that gear is replacable.
