Default clutch issues

Okay so today i am driving my car home from my parents house; I pull out onto the road and get on it pretty hard through first. When i go to shift into second gear it feels very knotchy. I slow down at the light and start to 'attempt' to downshift. It requires a lot of effort to downshift. I come to a complete stop and the light and am not able to get into first gear. I use a large amount of force and slam it into gear. I limp the car home. Check the clutch fluid, its full (a little dirty, but full).
With the car off i am able to shift into gear with ease ( no blockage )
If the car is on and i attempt to shift into gear the car wants to move, whether its in first or reverse. Also there is a 'clunk' and lurch feeling.
I decide to adjust the clutch as i feel that it is not completely disengaging. I pull the rod toward the driver seat as far as it goes. The problem still persists.
The master cylinder doesnt appear to be leaking. If the slave was the problem i would not have resistance in the clutch, and the clutch feels as stiff as it always has.

Any advice on what to do next??

If you hold the pedal down while continuously shifting between 1st and reverse and it clunks every time, then the clutch is dragging. If the hydraulics are not leaking and are bleed properly and the pedal is adjusted correctly (it should engauge the clutch at about half the pedal travel), then you need a new clutch because either the disk is warpped, or the pressure plate is damaged. Let us know what you find.
