That is the shift rod bushing, not spacer. It belongs in the housing not on the rod. It has popped out of where it belongs. If it continues to pop out, roughen up the surface of the hole it belongs in, put a VERY THIN coat of JBWeld on the the roughened walls and put it back in place. You can even spread it open just a TAD before tapping it back into place to help secure it.
And no, you are not missing bushings in all the other holes.
