You aren't running Boost Compensation or anything else like that? If so, you need to add on the additional pulsewidth from the compensation to get to your IDC value that the AEM is showing you.
fuel value * mSecbit * boostcomp% = 250*.000040 * 2.00 = 0.02
(assuming boost compensation of 200% at 1 Bar and other things constant) = 100% IDC.
If you need more fuel before compensation, or don't want to run boost comp then you need to use higher microsecond/bit numbers. You can't get to 100% IDC if msec/bit is too low to support it.
fuel value * mSecbit = 250*.000080 = 0.02
...and really you don't want to run much above 85% IDC because your injector coils may overheat and fail.
